Portal documentation fully refreshed

The project documentation has been reviewed in full so that every document matches the running code. The set covers installation, architecture, database schema, caching strategy, security, testing and the API reference.

Every page now has a cross-reference section and runnable example commands. Contributors are encouraged to read the coding guide before submitting changes.
